Abstract

1. CONSIDER THE FOLLOWING CASE.

A 59-year-old man had been more tired than usual for a couple of months. Two days before admission he ran a fever up to 39 °C. The next day the family doctor diagnosed oral thrush and prescribed treatment for it. On the day of admission he developed weakness of the right arm and problems with speech. He had travelled extensively in South America and Asia. Examination showed mixed dysphasia and a mild right hemiparesis. The unenhanced CT scan is shown in Fig. 1. There was no change after injection of iv contrast. What is the diagnosis?

A 18-month-old boy became tearful and anxious over the course of an hour. On examination he was poorly attentive but made eye contact. His skin was dry and red. The body temperature was normal, the heart rate 140 per minute.
