Abstract
1. Consider the following case.
A 61-year-old man woke up one morning with the feeling that his tongue was swollen. Also his speech was a bit slurred and he had difficulty swallowing. Two days before he had had a left-sided headache, which was unusual for him. Otherwise his medical history was unremarkable. The relevant signs are shown in the two photographs (Fig 1 and 2). What is your diagnosis?

2. What is the cause of this woman’s symptoms?
A 57-year-old woman presented with a three year history of parkinsonism on the right side of her body and speech difficulties. Figure 3 shows her axial T1 MR images.
