Imagine if James Parkinson had submitted his monograph, ‘The Shaking Palsy’ for consideration for publication today. Likely it would be rejected, maybe without peer review. He described only six patients, three of whom he simply observed walking on the street. Much of his description of the longitudinal course derived from a single case.1 One can imagine the editors asking, where is the evidence?
