    I was given this memorable paper (How To Swim With Sharks: A Primer*) by an American cardiology trainee in 1982 and it has been an occasional guide ever since. The author is given as ‘Voltaire Cousteau’1 though appears to have been Richard J Johns, a Professor of Biomedical Engineering at Johns Hopkins University and Hospital, Baltimore. It may be a complete coincidence that many of his other publications are neurological.
